At your first appointment, you and I will devote approximately 30 minutes to complete the following documents. We will also discuss your preferences and goals.

Initial client intake form
Office procedures

At each subsequent appointment you'll be asked to fill in the following form to communicate any changes in your health status and to indicate the specific areas you wish addressed or avoided in the day's massage session. If there are no changes in your health, completion can take less than 30 seconds.

Client update form

During and after each appointment I will complete the following form to record the details of the day's work. The information will be kept electronically for reference at future appointments.
